Through initiatives like YouthHealthConnect, we strive to bridge gaps, foster inclusivity, and pave the way for healthier, safer futures for our community's youth.
Supported by the Robert Wood Johnson Foundation through the Systems for Action National Program, one of our current projects focuses on the Systems Alignment to Optimize Health Services for Youth. Its primary objective is to engage multi-sector partners in developing, implementing, and assessing the feasibility of an integrated system of healthcare and public health programming for youth experiencing homelessness in and around Hennepin County, Minnesota.
For Youth-serving Agencies
The Minnesota Department of Human Services (DHS) has prepared example minor request forms to support youth-serving agencies in meeting licensing requirements. These forms can help streamline the process and ensure compliance with state regulations.
